Blue Shield of California has extended its relaxed participation requirements through December 31, 2026. Eligible small groups with at least five enrolled employees may qualify with 25% participation for medical, dental, vision, and life plans. The promotion applies to eligible off-exchange and mirror plans, subject to standard eligibility, enrollment, underwriting, and documentation requirements. At least 25% of eligible employees must enroll in a Blue Shield medical plan, and employees covered under another plan offered by the same employer are not considered valid waivers.

Blue Shield Refusal of Coverage forms are required for eligible employees who decline enrollment or enroll with an alongside carrier. Blue Shield must also be the sole carrier for dental, vision, and life coverage, while additional restrictions may apply when medical coverage is offered alongside another carrier.
