Dickerson's carriers' appointment information

Please have the agent click here and follow the step by step instructions. Unfortunately, Anthem doesn’t give Dickerson access to view the agent’s paperwork, therefore, if the agent has any questions, they must contact Anthem’s Licensing and Credentialing department for assistance at (800) 686-8279.

There is a $30.53 appointment fee. The agent will receive a direct email notice from Licensing and Credentials (Anthem’s unit) with the details where to send the live check.

Please have the agent click here to download the required forms. Once completed, please forward the completed forms to our customer service unit at CustomerService@dickerson-group.com. The agent can contact our unit at 323-796-8005 with questions or concerns.

Download and complete the Cal Choice Privacy Agreement, Direct Deposit Authorization, Broker License Form and Administrator’s Agent Agreement. Once completed, please forward to our customer service unit at CustomerService@dickerson-group.com along with a copy of the agent’s CA license. The agent can contact our unit at 323-796-8005 with questions or concerns.

To become appointed with Covered CA, please have the agent elect one of the following options:
1. Email: Agents@coveredca.gov
2. Agent Support Unit: 877-453-9198 – To request the contract to be emailed or faxed.

Please have the agent click here to become appointed. In order to contract with Health Net, the agent will need to have written or assumed their first piece of Health Net business. Upon writing or assuming the first Health Net group or individual contract, the agent will need to complete, the following documentation through the Health Net Broker Self Service Web Tool.
– Health Net of California, Inc. and Health Net Life Insurance Company Agent/Broker Agreement
– Web registration
– Direct Deposit Verification form
– Online W-9 form
– Proof of Errors & Omissions (E&O) insurance coverage

The name on all your documents, including the appointment paperwork must match. This means, if you are requesting an agency appointment, the agency name must be on the license, E&O, and W-9. Same for an Individual appointment, it should reflect the agent name on the license, E&O and W9
